Client Forms

As a new client, you will need to fill out paperwork to get started. At Phynix, we offer the opportunity to complete your forms online through Hushmail. We will send an email with your forms once we schedule your first visit, the evaluation. Hushmail encrypts all correspondences so that your privacy remains protected throughout your experience with us!
